The website provides general information about People Check’s occupational health, compliance, training, weight-management, primary-care, and Direct Primary Care services. Website content is not medical advice, diagnosis, treatment, legal advice, or a substitute for consultation with a qualified professional.
No provider-patient relationship
Viewing this website, sending an inquiry, or submitting a form does not create a provider-patient relationship and does not guarantee acceptance as a patient or member. A provider-patient relationship is established only through People Check’s applicable registration, consent, and clinical processes.
Emergencies and time-sensitive needs
This website is not monitored for emergencies or urgent medical needs. Do not use a website form to request emergency assistance or time-sensitive medical advice. Call 911 or seek emergency care for a medical emergency.
Forms, scheduling, and enrollment
A CPR training request does not reserve a class seat. A DPC interest request does not enroll a person in Direct Primary Care. Appointments, classes, services, eligibility, enrollment, deposits, and pricing are not confirmed until People Check communicates that confirmation directly.
Accuracy and availability
We work to keep website information accurate and current, but services, hours, availability, eligibility requirements, pricing, and other details may change. Please contact the appropriate People Check office to confirm information that is important to your decision or visit. We do not guarantee uninterrupted or error-free website operation.

You agree not to misuse the website, interfere with its operation, attempt unauthorized access, submit false or unlawful information, introduce malicious code, impersonate another person, or use the website in a way that violates applicable law or the rights of others.
